Fire Incident Report: Requestor may come to the Libertyville Fire Dept. Station No. 1 during business hours OR a written request may be sent to 1551 N. Milwaukee Avenue, Libertyville, Illinois 60048 with the following information: date of incident occurred; incident number (if known); location of incident occurred; name of occupant; and your reason for request of copy of incident. Note: There is a copying fee of $2 /page.

Fire Investigation Report: A fire investigation report will be released only to those individuals who fall into one of the following categories: Occupant/Owner, Insurance Company or Attorney. Requestor may come to the Libertyville Fire Dept. Station No. 1 during business hours OR a written request may be sent to 1551 N. Fire Ambulance Report: The patient or legal guardian for the patient must come to the Libertyville Fire Dept. Station No. 1 and show an ID picture and complete a HIPAA Patient Request for Access form during business hours OR patient’s attorney may submit a subpoena.
